I hereby authorize any repair work to be done along with the necessary materials in advance of the service. All Care Power Equipment and its employees may operate my equipment for the purposes of testing, inspection, or delivery at my risk. Full payment is expected upon completion of services rendered on said equipment. Failure to provide payment is an acknowledgment of a lien on the said equipment to secure the number of repairs thereto. Any unpaid equipment left for more than 60 days with non-payment is subject to being considered abandoned and is able to be sold by All Care Power Equipment under Massachusetts property abandonment law. It is also understood that you will not hold All Care Power Equipment responsible for any loss or damages to equipment in case of fire, theft or any other cause beyond All Care Power Equipment's control.
